Show the delivery-note number, order or PO reference, sender, recipient and delivery date.

Record the actual delivery
List item descriptions and quantities delivered rather than simply repeating ordered quantities when they differ.
Use notes for shortages, substitutions or visible condition issues where appropriate.
Plan for confirmation
Some workflows include recipient name, signature, date or delivery-status notes. Requirements differ by organization and jurisdiction.
